Switching workbook extension requests

How to request an extension for completion of a Switching on-the-job workbook

Eligibility

  • Extensions will be granted for extenuating circumstances, eg. illness, secondment to another role, etc.

Time Allowed

  • A maximum extension of one (1) month will be allowed.

Making an Application

  • An application for an extension must be submitted by your Formal Leader prior to the due date.
  • Applications for an extension should be submitted by your Formal Leader by email to the PTSWA Support Services team:

  • The email subject line should read: Name, Pay# / NAC#, PTS Course – Request for Extension
  • The email body must include:
    1. the reason for the request;
    2. the original due date for submission; and
    3. the requested submission due date
  • All applications must include details of the activities completed to date (this may be the summary sheets from the workbook).
  • If approved, an email will be received from PTSWA acknowledging the request and noting the new due date.
  • If declined, an email will be received outlining the reasons for refusal.
  • A record of the request and any approval for extension will be held on the student record and the due date will be updated in the PTSWA Training Administration System.

Submission of Workbook

  • When the workbook is submitted for assessment, a copy of the approval email must be included.
  • All evidence must be no more than six months old.

Please Note: No further extensions will be granted. If a workbook is deemed to be Not Yet Satisfactory after an extension has been granted, this will be the final course result recorded. There will be no further opportunity to correct non-compliances or submit additional evidence.
